Companionship for Mrs AS
Mrs AS was housebound but is sociable and likes to chat. Due to her not being able to get out and about, it was important for her to have companionship support at home. She wanted us to arrange for someone to visit her twice a week for two hours.
What we did
Our first priority was to find out a bit on Mrs AS’s likes and dislikes, as well as what she wanted to achieve from us. By listening to what her needs and preferences were, we found out that she particularly likes to knit and play scrabble. We matched her with a carer who is chatty and friendly, who also likes playing scrabble. The visits are varied, and Mrs AS is in control as to what activities she wants to do within the time allocated.
Outcome
Mrs AS is now enjoying the carers visits and is so much more confident and happier at home.
Love Your Independence
Person-centred care that includes the cat!
Mrs JU is a lovely independent lady who lives with her cat and wants things done in a certain way. This is important to her. Prior to meeting her, she had a fall and this limited her mobility and affected her ability to do certain things herself. She needed support to enable her to live in her own home, as independently as possible.
What we did
We went and had an initial assessment with Mrs JU, taking into account all her preferences, wishes, values and choices. As an example, the care plan for our client was even tailored towards this including support for her cat, as this was important to her.
Outcome
Mrs JU is happy that she is at the centre of care and all her needs are being met as desired.

Care that transforms
Mrs UP commenced at a point when she was at a critical stage. She was discharged from hospital with End of life medication, syringe drivers and other equipment as she was classed as end of life Dementia. She was given just a few months to live.
What we did
Due to our approach of having a multi-disciplinary team with highly trained nurses and carers we were able to provide a complete nursing and care support service. Once Mrs UP was discharged, we saw she had pressure sores, so we organised for an air mattress for her. An air mattress significantly helps pressure sore recovery which can, if left untreated, lead to further hospitalisation. The care she received included supporting the family on how to manage her Dementia and the importance of the prescribed medication. We also discovered the family were paying for items relating to personal care. We were able to ensure they received these free through the NHS. Additionally, we worked in unison with the multidisciplinary team (MDT) on her progress, created a new care plan and implemented this to provide her with the best support possible.
Outcome
With our loving care, Mrs UP recuperated remarkably and was no longer considered to be End of life. This was largely owing to support from our Dementia specialists. She remains healthy and happy with her family and our carers read interesting books and sing to Mrs UP which is what she loves to do

Improving outcomes
Mr S had a number of health issues including Diabetes and Dementia. The Diabetes wasn’t under control and does have frequent falls due to frailty, leading to many stays in hospital. Due to his Dementia he often became agitated. He wanted to go out on his Scooter but his son was concerned he would injure himself or be unable to find his way home. His son was finding it difficult to cope on a day-to-day basis.
What we did
We initially held a multi-disciplinary team meeting which included nurses, his GP and social worker to discuss the best approach and solution for Mr S. Due to falling and mental health state the multi-disciplinary team reviewed Mr S’s wellbeing and felt that live in care was the only option that would work for him. We put a care plan in place that took into account both his nursing care needs as well as his social care needs.
We were able to find a Dementia trained carer with 13 years’ experience to provide the live in care Mr S needed.
Outcome
They have a fantastic relationship and Mr S is living the life he wants to live. Additionally, there have been no more hospital admissions because his diabetes is kept under control and our amazing carer accompanies him on all his visits outside of the home.
